Why every caregiver should have one
In an emergency, the people helping your parent need the basics fast: conditions, allergies, medications, doctors, insurance, and who holds power of attorney. Scrambling for that in a crisis costs precious time. This one-pager puts it all on a single sheet, in plain language, so it's ready before you ever need it.
A note on privacy: what you enter is organized into the summary and then forgotten. We don't store it. Leave out full ID numbers, note where the card is kept instead.
